Every individual over the age of 13, who is being seen at this clinic has the right to his or her privacy. The clinician will not release information to client or anyone else without an ROI signed and dated, no more than six months of date signed. If youth older than 13, would like his or her parents to have access to his or her records, he or she must sign an ROI to provide the clinician permission to release said records. At the time of records request there will be a fee of .10 cents per-page, to assist in paying for ink, and paper charges.
Clinician will not discuss any client’s information with outside agencies, or individuals without permission of client first. Unless it is an open/new CPS case or call, adult/elder abuse, threats to any persons, or possessions, and if concerns about possible suicide attempt or threats. If at any time client feels his or her confidentiality has been broken, please feel free to contact the clinician, OMBUDS, or DSHS to file complaints. Confidentiality does not extend to conversations in the waiting room with friends, loved ones, or reception. There is no way to control what others say or hear while waiting in the waiting area, or during group counseling sessions.
